Audit Observation

Definition

An audit observation is a condition noted by an auditor during a compliance assessment that does not rise to the level of a formal finding or nonconformity but represents an area where the organization could improve its controls, processes, or documentation. Observations serve as early warnings -- areas that could become findings if not addressed.

Observations vs. Findings

Severity

Observation: Does not constitute non-compliance
Finding: Represents a clear deviation from requirements

Required Action

Observation: No formal corrective action required
Finding: Must be formally remediated with documented corrective action

Audit Impact

Observation: Does not affect audit opinion or certification
Finding: Can result in qualified opinion or failed certification

Follow-up

Observation: May be reviewed in next audit cycle
Finding: Must be resolved before next audit, often within defined timeframe

Why You Should Still Address Observations

Observations often become findings in the next audit cycle if left unaddressed

Addressing observations proactively demonstrates compliance maturity to auditors

Observations reveal areas where controls are weakening before they fail completely

Fixing observations is typically less effort than remediating the findings they could become

A pattern of unaddressed observations signals systemic issues to experienced auditors
